Peacefull Havens helps Michigan residents understand eligibility for the Home Help Program and start the process without confusion. Home Help is funded by Michigan Medicaid and can pay for personal care so eligible adults can remain safely at home. Peacefull Havens does not approve or deny applications. We help you move through each step.

Eligibility requirements at a glance
- Michigan resident living in a home or apartment, not a nursing facility
- Enrolled in Medicaid before services can be authorized
- Need for hands on help with daily activities
- A required face to face assessment with an adult services worker
Activities reviewed for eligibility
Activities of Daily Living: bathing, dressing, toileting, mobility, eating, grooming
Instrumental Activities of Daily Living: meal preparation, light housekeeping, laundry, shopping for essentials when appropriate
Required forms
Adult Services Application (DHS 390). Completed and signed by the applicant or their legal representative. This form starts the case with the state.
Medical Needs Form (DHS 54A). Completed and signed by a licensed health care professional. It confirms that help with daily activities is related to a medical condition, and must be current, signed, and dated.
How eligibility is determined
